Did you know that you could save time and money by using the mail order prescription drug program?

Mail service allows you the convenience of receiving a larger supply of covered maintenance drugs for a lower co-payment then you would pay at your retail pharmacy. The mail service pharmacy is available for those who are taking stabilized dosages of covered maintenance drugs on an ongoing basis for treatment of a long-term condition, such as high blood pressure.

To obtain drugs through the mail service (Express Scripts), please follow these simple steps.

1. Complete Patient Profile Questionnaire on the mail order form.
2. Select the payment method, ie. Credit card, check or money order.
3. Send the completed questionnaire, your original prescription(s), and the applicable mail service copayment(s) for each drug to the mail service pharmacy.

Please allow up to 14 days for delivery from the time you mail your prescription. If you have a question about the copayment of your prescription or the status of your mail service prescription, please call Express Scripts at (800) 544-6962. This is a 24 hour customer service number.
